Volunteer Driver/Chaperone Application Checklist
 
To volunteer as a driver or chaperone at Mammoth Unified School District, you must complete the following steps:
 
  1. Complete a Volunteer Application

  2. Complete a Principal's/Director's Authorization including meeting and dates principal/athletic director signature

  3. Complete Driver forms for district and private vehicles

  4. Submit a current TB Test Assessment

  5. Return all completed and signed forms, in person to the MUSD Office

  6. Once your packet is complete, we will require a LiveScan - fees outlined in application
 
After you successfully complete the above steps, you will be placed on the MUSD Approved Volunteer Driver/Chaperon List.
