Volunteer Driver/Chaperone Application Checklist
To volunteer as a driver or chaperone at Mammoth Unified School District, you must complete the following steps:
- Complete a Volunteer Application
- Complete a Principal's/Director's Authorization including meeting and dates principal/athletic director signature
- Complete Driver forms for district and private vehicles
- Submit a current TB Test Assessment
- Return all completed and signed forms, in person to the MUSD Office
- Once your packet is complete, we will require a LiveScan - fees outlined in application
After you successfully complete the above steps, you will be placed on the MUSD Approved Volunteer Driver/Chaperon List.
