Immunization Requirements for School Entry
Vaccination is one of the best ways parents can protect infants, children and teens from 16 potentially harmful diseases. Vaccine-preventable diseases can be very serious, may require hospitalization, or even be deadly - especially in infants and young children.
Mammoth Unified School District wants to promote and support a healthy learning environment for all of our students, staff and community. We also want to ensure you have current information regarding immunization requirements that may impact your child. To enter school, parents must present a signed or stamped immunization medical record from a physician or clinic. Students are required to meet all state immunization requirements.
New Regulations starting July 1, 2019
New Immunization dose requirements for school admittance (TK/K-12)
The information provided below tells you what number of doses are required of each immunization at each grade level.
K-12 Admission
- 4 x Polio
- 5 x DTaP
- 3 x Hep B
- 2 x MMR
- 2 x Varicella
7th - 12th Grade
- 1 x Tdap
7th Grade Advancement
- 1 x Tdap
- 2 x Varicella
Vaccination Law Update and Elimination of Personal Beliefs Exemption
Under a new law known as SB 277, beginning January 1, 2016 exemptions based on personal beliefs, including religious beliefs, will no longer be an option for the vaccines that are currently required for entry into child care or school in California. Most families will not be affected by the new law because their children have received all required vaccinations. Personal beliefs exemptions (PBE) on file for a child already attending child care or school will remain valid until the child reaches the next immunization checkpoint at kindergarten (including transitional kindergarten) or 7th grade.
Beginning January 1, 2021, all new medical exemptions for school and child care entry must be issued through CAIR-ME. If any of these immunizations were not given to your child because of medical reasons, please bring us a medical exemption form issued using the CAIR-Medical Exemption website by your child’s doctor (MD or DO licensed in California).
