TK/K Enrollment - Physical + Oral Health Exam + TB Assessment
Health and Safety Code 124085- requires students entering Kindergarten or first grade in a public or private school in California have a comprehensive health examination. Each child shall present evidence that a physical screening examination has been done sometime during the 18 months before entering first grade. (A grace period of 90 days is allowed after entry to first grade). The “Report of Health Examination for School Entry” form (PM 171A) must be signed by a licensed health care provider.
Oral Health Assessment
California law (Education Code Section 49452.8) says every child must have a dental check-up (assessment) by May 31st of his/her first year in public school. A California licensed dental professional must do the check-up and fill out Section 2 of the form linked below. If your child had a dental check-up in the last 12 months, ask your dentist to fill out Section 2. If you are unable to get a dental check-up for your child, fill out the separate Waiver of Oral Health Assessment Requirement Form.
TB Risk Assessment
Effective February 1, 2026, in accordance with California Health and Safety Code Sections 121475-121520:
All new students enrolling in a TK–12 school must undergo screening and clearance for Tuberculosis (TB). To enter school, a Pediatric TB Risk Assessment is required, and/or a Tuberculosis Clearance Form must be completed by a licensed healthcare professional and submitted to your child’s school.
Tuberculosis is a bacterial infection that can spread through the air from one person to another when a person with active TB coughs, sneezes, or speaks. TB is a preventable disease, and the goal is to eliminate TB in Los Angeles County and California.
