California Public Records Act Request Form

Mammoth Unified School District is committed to transparency and providing the public with access to District records in accordance with the California Public Records Act (CPRA).

The CPRA recognizes that access to information concerning the conduct of the public’s business is a fundamental right. A public record generally includes any writing related to the public’s business that is prepared, owned, used, or retained by a state or local agency, regardless of its physical form. Public records may include documents, correspondence, emails, reports, photographs, electronic files, and audio or video recordings.

To help the District locate the records you are seeking, please describe your request as clearly and specifically as possible. When available, include:

The subject or type of record
Relevant dates or a date range
Document, report, or project titles
Names of individuals, schools, offices, or departments
Keywords or other details that may help identify the records

You do not need to explain why you are requesting the records.

The CPRA provides access to existing, identifiable records. It does not require the District to create a new record, answer questions, conduct research, or compile, summarize, or reorganize information in a form that does not already exist.

Some records or portions of records may be confidential or exempt from disclosure under state or federal law. These may include information protected by student privacy, personnel confidentiality, attorney-client privilege, or other legal protections. When appropriate, the District will redact protected information and provide the remaining disclosable portions of a record.

Submitting this form is a convenient way to make a request and helps ensure the District receives the information needed to begin its search. If your request is unclear or overly broad, District staff may contact you to help identify the records you are seeking.
